The Supportive Care Continuum

Saint Mary's Home Care Services, through the Supportive Care Continuum, also provides loving support and care to patients and families faced with progressive or life-threatening illness. The Supportive Care Continuum, staffed by a dedicated team of professionals under the direction of the patient's primary physician, attends to the needs of patients in the home, including emotional and spiritual well-being, symptom management, and physical comfort.

When appropriate, patients have the ability to transition to Saint Mary's Hospice care if that is their wish. Volunteers are also available to visit patients and to support the family.
